Health Care Cost
Reimbursement:

Members can receive up to $300 tax-free per Plan Year in reimbursement for prescription drugs or other  out-of-pocket health care expenses.

Dental Care:




                                                   

The maximum reimbursement benefit is $4,000 per Plan Year.  You and your family pay no deductibles.  Your family is eligible to receive up to $8,000 per Plan Year in reimbursements for preventative, diagnostic, basic or major services. 

Disability Benefit:

 $1500 monthly benefit for a maximum of three years for members who are totally unable to perform the duties of their profession as a result of illness or injury.
 

Hearing Aids:

Members and their spouses or domestic partners are eligible for up to $900 every two years for the installation or repair of a hearing aid and the associated hearing exam.

Legal Services:

Legal representation regarding: real estate, matrimonial, adoption, estate planning, personal injury and estate administration are available to members, their spouses or domestic partners at sharply reduced fees.  In addition, two hours of legal consultation or legal services are available at no charge each Plan Year. 

 Mammography Benefit:

$200 reimbursement per Plan Year is available to members, spouses or domestic partners.
 

Optical: 

Members, their spouses or domestic partners, and children will receive up to $250 per person per Plan Year in reimbursement for eye glasses, contact lenses and eye exams. 

Physical Exams:

Free annual physicals including laboratory tests for members, spouses or domestic partners.
 

Podiatry: 

Members and their spouses or domestic partners are reimbursed up to $5,000 per Plan Year for podiatric services.

Private Duty Nursing: 

Members and their spouses or domestic partners may be reimbursed for up to $5,400 per Plan Year for private duty nursing per hospital confinement.
